RESUMO

A definitive method to predictably treat propagated longitudinal fractures remains elusive. A proof-of-concept case report series documenting nonsurgical removal of propagated longitudinal fractures has shown up to 5 years of clinical and radiographic success. This installment of the case report series further documents these teeth with 7.5-year to 11.5-year recall evaluations. Three previously reported cases of fracture removal were followed at 11.5 years (1 case), 9.75 years (1 case), and 7.5 years (1 case) after treatment to determine the long-term clinical and radiographic success of nonsurgical fracture removal. This case report series has demonstrated 10-year success (clinically and radiographically) for the treatment of progressive longitudinal fractures. Propagated fractures can be nonsurgically removed and the iatrogenic defect repaired with long-term success providing a foundation for treatment and further investigation.

RESUMO

Extranodal mucosa-associated lymphoid tissue lymphoma is a rare oral disease. This case report describes a 71-year-old woman who presented for endodontic evaluation and treatment for swelling on the right posterior palatal. The case details the evaluation and diagnostic processes and treatment course. The case report draws attention to the importance of performing a biopsy, having proper sampling, and the additional steps sometimes required to arrive at a definitive diagnosis. This case report is the first to document a definitive low-grade B-cell mucosa-associated lymphoid tissue lymphoma mimicking endodontic pathology. The case shows the importance of proper examination, clinical imaging, and pathologic testing to arrive at a definitive diagnosis. It highlights that collaboration with other medical and dental specialties is essential for appropriate diagnosis and treatment and describes the "buttonhole" diagnostic strategy.

An apically propagated crown fracture has an unfavorable prognosis and typically leads to extraction of the tooth. Although novel treatments for the repair of crown/root fractures have been documented, the long-term predictability of the treatments remains unclear. Three previously reported cases of fracture removal were followed 3.5 years (1 case) and 5.5 years (2 cases) after treatment to determine the long-term clinical and radiographic effects of nonsurgical fracture removal. This recall case report series shows the ability of the fracture removal technique to provide long-term success (clinically and radiographically) for the treatment of apically progressive crown fractures. The fracture removal technique has resulted in the maintenance of teeth that would otherwise have been extracted as well as preservation of the site should extraction be required in the future.

INTRODUCTION: A major complication after endodontic treatment is persistent pain on biting or chewing. Although unsuccessful endodontic treatment can account for such pain, pain to chewing and biting can also be a result of an unsupported or propagated crown fracture. Crown fractures that extend apically result in deterioration of the localized periodontium and ultimate loss of the tooth. To date, no predictable treatment for apically extending crown fractures exists. METHODS: Three cases of nonsurgical excision of a propagated crown fracture and the repair of the subsequent iatrogenic perforation are presented. RESULTS/CONCLUSIONS: This case report series describes a novel treatment for apically progressive crown fractures that results in patient comfort and maintenance as well as improved periodontal status of the tooth.

RESUMO

This case report details the treatment and follow-up of an avulsed and malrepositioned maxillary anterior tooth. Treatment of the malrepositioned tooth included re-extraction and replantation as well as root canal therapy. Initial success at 1-year post-trauma was overshadowed by the development of external root resorption and a horizontal root fracture, which occurred between 1 and 2 years after trauma. The development of external root resorption can be attributed either secondarily to the original trauma or due to active orthodontic treatment.
